We offer both a medical and surgical weight loss program.- individualized for each person based on their needs, desires and tastes. We perform Laparoscopic Gastric Bypass, Laparoscopic Sleeve Gastrectomy and the Laparoscopic Band, again based on individual evaluation. 1. Please fill out the Weight Loss Program Questionaire found HERE.

2. Bring Questionaire to one of our Seminars or drop off at our office 2385 S. Melrose Dr., Vista, CA  92081.

3. Talk to your Primary Care doctor about weight loss programs and weight loss surgery and have them fax us a consult at 760-482-1316.

4. Call your health care insurance program and see if they cover weight loss programs and bariatric surgery.
